Initiating Service
Typically, physicians initiate referrals from their office, local hospitals, board of care & skilled nursing facilities. Referrals may also be generated by the patients families, interested persons or agencies.
After our intake coordinator receives the referral information, our professional nurse will asses the patient within 24 to 48 hours unless immediate care is required
